State law requires a certificate of acknowledgment and jurat to have the exact language set forth in the statute (Civil Code section 1189 and Government Code section 8202.) Forms with the correct language are on the Secretary of State website at http://www.sos.ca.gov/business/notary/forms/notary_ack.pdf and http://www.sos.ca.gov/business/notary/forms/notary_jurat.pdf.  The Secretary of State’s office has rejected documents with incorrect acknowledgments and jurats.  The 2014 Notary News on the Secretary of State’s website can be accessed at http://www.sos.ca.gov/business/notary/forms/notary-newsletter-2014.pdf
